Do I need to put a social security number on the application form?

Yes, social security numbers are required on all applications.  Applicants who do not provide their social security number will experience delays in the processing, or possibly the denial, of their application.  If a child applicant has never been issued a social security number, a statement to that effect must be submitted along with the application.
